Navigate to Device Management à SIM Management
- Select "Verizon IoT (SIMETRY)" as the carrier account.

- Select the "Perform Action" icon to fill in the required fields of the activation form. *Please note that you will have the option to import a Microsoft Excel file to fill in the blank form. You also have the ability to receive either an SMS or email notification once the action has been completed.

- This form will require you to fill in the following fields.
- Column A: ICCID
- Column B: Select "SKU".
- Column C: "VZW080000460061" for standard activations or "VZW120003650017" for CAT-M1/NB-IoT Activations.
- Column D: The service plan code that you wish to activate with.
- Column E: Use one of these two zip codes: 77354 or 77024.

- Enter a plan code:
- NEWTIEREDTXTSTATIC *Add a public static IP address.
- NEWTIEREDSTATICNOTX *Block SMS and add a public static IP address.
- NEWTIEREDDATATXT *Default plan code.
- NEWTIEREDDATANOTXT *Block SMS.
- Please leave all other fields blank on the activation sheet.
- Once the form has been submitted, you will be presented with a unique transaction ID. You can track the status of your request by navigating to Device Management à Action Status.
